Carers Support

Adding alternative contact details for Vulnerable Patients that you care for

If you are a carer for one of our patients (official or unofficial) and feel that the person you care for could become confused or distressed when receiving communication from the practice or our clinicians, please let us know.

If the person you care for has capacity to provide consent, we can add your contact details to their medical record to enable you to manage communication on their behalf.

If the person you care for does not have capacity to consent, please speak to us for further advice on how we can help.
