Medical records hold information about you. They are also sometimes called health records.

You have separate records for the different NHS services that you use, including your GP surgery, Hospital, Dentist or Opticians.

All these records are held seperately and so depending on what information you require, you may have to contact that specific providerdirectly.

Here at the surgery, we only hold you GP records.

How to Access your GP Record

Your GP record holds your core medical information which includes current and past medical conditions, allergies, medication and a full medical history of all instances where you have visited you GP surgery. 

Most patients will automatically be given access to their GP record online if they have provided us with a valid email address. This will include letters, test results and consultation notes which will be viewable from the date they are given access.

Some people may also have access to information added to their GP record before this date.  This can be obtained by applying for full online access.  

There are several ways to access your GP record.

  • You can access your GP record if you have (or register for) an NHS account.
  • If you do not have access to the internet, you can nominate a family member or someone you trust, to have access to this on your behalf.
  • You can request details from your medical record to be provided to you via email or in paper form.  To do this, you will need to request this in writing and specify exactly what information you require.

Accessing your Records via an NHS Account

You can access your GP record by logging into your NHS account using the NHS app or NHS website.

If you do not currently have an NHS account, you will need to register for online services (see link below) and provide the relevant identity checks. 

You will then need to ask your GP surgery to activate online access to your full record, otherwise you will only see your medicines and allergies.
