The practice records data such as height, weight, smoking status, alcohol consumption and ethnicity in order to maintain up to date and accurate medical
records for all its patients. If you are not a regular attender at the surgery you may be asked this information during your consultation with a clinician, even if it does not relate to the problem you originally came in for.

Medical Students

Fifth year medical students, in their final year pre qualification, are working with us August to March each year. If you do not want a medical student to be present at your consultation, please let reception staff know.

Non urgent Queries

This provides the facility for patients to email GPs, Advanced Practitioner Nurses and Practice Nurses with a routine query

If you have a routine query that does not require an immediate answer, we aim to respond within 48 hours.

NB please note:

  • Your query will be filtered to go to an appropriate member of staff – you can identify a GP/Nurse but the response may not necessarily be from that particular member of staff
  • Identify whether you are a patient at Hadfield or Glossop
  • You can also use this facility to enquire about private medical reports

How does this work?

  1. Follow the link to online query form and then enter your name and email address in the box at the bottom of the page.
  2. An email will be sent to you with a consultation ID
  3. You can then go back to the page and enter your ID and question
  4. The question will be sent to the practice who will respond accordingly
